Im running on the same issue.
No API key fields under registration page or user control panel.
Im using the latest phpBB (3.0.11).
Just for a sidenote: I have adjusted some conflicts while installing the mod (1 to be exactly) but still didnt work.

Try to use the EFT format (latest version) or exporting from Pyfa (EFT style too).
The fitting may be missplaced if you have any extra linebreak.

You need to fill the api fields. This error is the validation of those fields.
With 3.0.11 I got some errors that you can avoid doing some manually editing at the configuration file that holds all the code info and updating what is different. But I dunno if that will work okey after this (probably will).

The main reason about accounts being inactive is due to lack of error handling (I'm not blaming the dev - he actually have done a really good job).
If any **** happens while trying to contact the eve api site, the mod will inactive the account.
Try to do an specific catch for incorrect apis and disable just in this case, not if you receive any kind of error!
I will try to modify this later on my own but im without much time these days
